Indications:

Ventricular arrhythmias, Supraventricular arrhythmias, Rapid control of arrhythmias,

Contraindications:

Preexisting 2nd or 3rd AV block without pacemaker. Right bundle branch block associated with a left hemiblock without pacemaker. Recent MI, cardiogenic shock.

Adverse reactions:

Dizziness, visual disturbances, lightheadedness and other CNS effects, nausea, vomiting, headache, tremor, peripheral neuropathy, corneal deposits, blurred vision or seeing spots, pulmonary fibrosis, pneumonitis, chest pain, ataxia, skin reactions; liver function disturbances. Potentially Fatal: Ventricular or supraventricular arrhythmias, congestive heart failure, 2nd or 3rd ° AV block, sinus bradycardia, sinus pause, sinus arrest.

Interactions:

Other antiarrhythmics or arrhythmogenic drugs, β-blockers; antimalarials; H2-antagonists, urinary acidifiers/alkalinisers, bone marrow depressants, digoxin. Effects of flecainide may be increased by concurrent admin of amiodarone, cimetidine, cisapride, propanolol and ritonavir. Not advisable to use flecainide with drugs that have negative inotropic effect such as verapamil or disopyramide unless benefits outweigh risks.

Warnings:

Patients with pacemakers. Congestive heart failure; sick sinus syndrome. Monitor ECG, blood counts and plasma concentrations. Not for control of symptomatic arrhythmias especially in patients with history of MI. Correct electrolyte imbalances before initiating therapy. Pronounced hepatic impairment. MI with associated left ventricular function impairment. May affect ability to drive or operate machinery. Elderly, child, pregnancy, lactation. Pregnancy: Category C
